The information requested depends on the planning path you choose. It may include:

  • Disability insurance planning information, including occupation, work arrangement, earned-income range, existing disability coverage, and desired monthly income protection.
  • Long-term care planning information, including whether you currently receive care, whether you have been diagnosed with a cognitive condition, and Medicare or Medicaid enrollment.
  • Other information you choose to provide when communicating with us.

Please do not submit Social Security numbers, financial account numbers, medical records, insurance identification numbers, or other information that the intake does not request.

Information collected automatically

Like most websites, the service used to host this website may automatically receive limited technical information, such as an IP address, browser and device type, requested pages, and the date and time of a request. We do not currently use advertising cookies, behavioral advertising tools, or website analytics tools.

The website uses browser session storage to remember the coverage path you selected during the current browser session. The website does not use that selection to track you across other websites.

Google address suggestions

The address field uses Google Maps Platform and Places services. When you type in that field, the address text is sent directly to Google so Google can return address suggestions. Google may also use cookies or similar technologies and receive device, browser, or usage information when its services load. Google handles that information under the Google Privacy Policy. You may enter your address manually if suggestions are unavailable.

How we use information

We use submitted information only as reasonably necessary to:

  • Review and respond to your insurance inquiry.
  • Allow Steven J. Rzezuski to contact you manually by telephone or email about the coverage you selected.
  • Deliver your completed intake to Steven J. Rzezuski by email and send a separate confirmation email to the address you provide.
  • Understand your stated insurance-planning needs and prepare for a conversation with you.
  • Operate, secure, troubleshoot, and improve the website and intake experience.
  • Comply with legal, regulatory, licensing, supervision, recordkeeping, and fraud-prevention obligations.

Completing the intake is not an application for insurance and does not authorize us to obtain medical records, consumer reports, or information from other people. If any later application requires additional information or authorization, it will be requested separately.

If you later ask to apply for coverage, any carrier disclosure and authorization will be handled as part of that separate process.

Telephone and email contact

By selecting “Submit intake,” you request a response to your inquiry and consent to manual telephone calls or emails from Steven J. Rzezuski on behalf of National Financial Network about that inquiry. Your completed intake is transmitted to Steven J. Rzezuski by email, and the website sends an automated email confirming that the intake was received. This website does not request consent for automated calls, prerecorded calls, text messages, or unrelated marketing campaigns. You may ask not to be contacted again at any time.
