These terms govern your use of this insurance planning website and its Life, Disability, and Long-Term Care intake paths.

Acceptance of these terms

These Terms of Use (“Terms”) are an agreement between you and National Financial Network (“NFN,” “we,” “us,” or “our”) concerning this website, which is presented by Steven J. Rzezuski on NFN’s behalf. By accessing or using the website, you agree to these Terms and acknowledge the Privacy Policy. If you do not agree, do not use the website or submit an intake.

Eligibility

You must be at least 18 years old and legally able to agree to these Terms. The website is intended for people located in the United States. Insurance products and services are available only in jurisdictions where the relevant producer and provider are properly licensed or otherwise permitted to act.

Purpose of the website

The website provides a preliminary intake for people interested in life insurance, disability insurance, or long-term care insurance. Its content is general educational and planning information. The intake is intended to help Steven J. Rzezuski understand the topics you wish to discuss and respond to your request.

The website does not issue insurance, bind coverage, provide an insurance quote, determine eligibility, or complete an application. No insurer is obligated to offer coverage. Product availability, premiums, terms, and eligibility are determined through a separate application and underwriting process and may vary by insurer and jurisdiction.

No professional or individualized advice

Website content and intake questions are not legal, tax, medical, investment, or individualized insurance advice. They are not a recommendation to purchase, replace, surrender, or modify any product. Nothing on the website is an offer to buy or sell securities or provide investment advisory services. Consult qualified professionals about your circumstances before making a decision.

No client, advisory, or insurance relationship

Using the website, completing an intake, or communicating in response to an intake does not by itself create an insurance contract, brokerage relationship, investment advisory relationship, fiduciary relationship, or other professional engagement. Any such relationship requires the applicable separate agreements, disclosures, authorizations, and acceptance procedures.

Your information and electronic submission

You agree to provide information that is accurate, current, and yours to provide. Do not submit information about another person without lawful authority. Do not submit Social Security numbers, financial account numbers, medical records, insurance identification numbers, or other information the intake does not request.

Selecting “Submit intake” is your electronic action confirming the disclosure shown with the button. It requests and authorizes manual telephone or email contact about your insurance inquiry and authorizes transmission of the completed intake to Steven J. Rzezuski by email. The website also sends an automated email confirming receipt. It does not authorize automated calls, prerecorded calls, text messages, or unrelated marketing campaigns, and it is not an insurance application or electronic signature on an insurance contract.

Privacy

Our Privacy Policy explains the categories of information handled through the website, the purposes for which it is used, and your choices. The Privacy Policy is incorporated into these Terms.

Google Maps services

Address suggestions are provided using Google Maps Platform and Places services. Your use of those features is also subject to the Google Maps/Google Earth Additional Terms of Service and the Google Privacy Policy.

Permitted use

You may use the website only for lawful, personal purposes consistent with these Terms. You may not:

  • Use the website to impersonate another person or submit false data.
  • Attempt to gain unauthorized access to the website, its systems, or information associated with other users.

Nothing in these Terms excludes liability that cannot lawfully be limited or excluded.

We may update these Terms when the website, our practices, or applicable requirements change. The effective date above identifies the current version. Your continued use after an update means you accept the revised Terms to the extent permitted by law.
